Surveys are only for patients with biochemical confirmation of CPT II deficiency. Diagnosis is confirmed by enzymatic analysis of muscle tissue or cultured skin fibroblasts, or by mutation analysis. Incomplete surveys will not be tabulated. 

For the purposes of this survey, a major episode is defined as creatine kinase (CK) in excess of 1,000 and/or the presence of dark urine. If you have not experienced major episodes, omit questions #1, #8 and #9.
